Hospice Aide

Gosnell - Per Diem

The Hospice Aide (HA) is a paraprofessional responsible to provide direct care to patients in the hospice house to improve quality of life and promote comfort. The HA performs tasks required to help maintain and restore the physical, social, and emotional health of patients, under the direct supervision of a registered nurse.

Resource Nurse

Gosnell - 36-Hours (Friday, Saturday and Sunday 10am-10pm)

The Resource Nurse works as part of the clinical management team and is responsible for assisting and coordinating the day-to-day clinical activities at GMHH and assisting with facilitating communication and providing leadership to the GMHH staff. The Resource Nurse is the primary supervisor on site during certain parts of the day. Performs admissions, manages medication ordering and supply, reviews and updates patient care plans and conducts IDT.

The Social Worker is responsible to provide quality, cost effective social work care in a competent holistic manner for patients in their place of residence, through assessment, planning, implementation and evaluation of social work care, to empower patients and families at end-of-life to achieve maximum levels of comfort and independence.

The RN Case Manager has the primary responsibility to administer and coordinate high-quality, cost-effective hospice care for home care patients within an interdisciplinary team model. They oversee the interdisciplinary hospice team to ensure that patients’ physical, emotional, and spiritual needs are met.
