Prerequisites for Employment
All Applicants
1. Favorable Background Investigations, including:
Criminal record in the states that the individual has lived or worked in the past 7 years
Social security number verification
Verification of highest level of educational degree attained, if required for the position
Medicare Sanction/Exclusion check
MaineCare Sanction/Exclusion check
National Sex Offender registry check
2. Documentation of positive work references from a minimum of two (2) professional references, when possible.
3. Proof of current and previously held professional licensure (in all applicable states), certification, or other training, as required for the position.
4. Proof of immunization (tests and immunizations will be provided through HSM, if necessary):
Baseline TB test before employment begins; unless documentation of negative test within previous 12 months received; additional testing may be required annually or more frequently as the agency deems necessary, and
Proof of immunization or documented immunity for Hepatitis B, or signed declination statement, and
Proof of immunization or documented immunity for MMR, and
Proof of immunization or documented immunity for Varicella, and
Proof of seasonal influenza vaccine or documented medical exemption.
5. Documentation of negative substance abuse testing.
6. Paid Employees: Proof of eligibility to work in the United States (I-9 form must be completed with proper supporting documents on or before the first day of employment).
All Applicants Required to Drive
1. Prior to employment, Motor Vehicle Records (MVR) will be checked to ensure that they have a valid driver’s license and meets the requirements of HSM’s liability criteria. Minimum MVR requirements for employment include:
No more than three moving violations during the past three years, and
No more than one chargeable accident during the past three years, and
No license suspensions or revocations within the past seven years, and
No major convictions (driving under the influence of alcohol or drugs, reckless driving, etc.) within the past seven years, and
No leaving the scene of an accident conviction, and
No failure to report an accident conviction, and
No incidents involving death conviction.
2. Copy of valid Driver’s License issued by the state in which they are a legal resident.
3. Proof of current auto insurance coverage that meets Maine Law.
4. Copy of current vehicle registration.
