Home Hospice

Most people receive hospice care wherever they call home, whether in their home, assisted living facility, or nursing home. Home hospice care is a unique model of care where nurses, aides, and other team members come to you, and supplies, medications, and other support services are delivered directly to your door. Patients receive regular nurse visits, and these other optional services:

  • Help with managing medications

  • Hospice aide visits to assist with personal care

  • Social Worker assistance with resources and counseling about what to expect

  • Non-denominational chaplain visits

  • Volunteer assistance and companionship

  • Grief support before, during, and after death
